South Peninsula Hospital's Community Health Services is a network of programs designed to meet the specific needs of the elderly and disabled people choosing to remain in their community.  We work closely with families, physicians, and other agencies to provide an individualized plan for each client.

SPH is recognized by the American Diabetes Association
for its Diabetes Self-Management Program.


Services include:

Home Health:
Community Health Service's medicare and medicaid certified home health agency provides skilled nursing, physical therapy, medical social work, certified nursing assistants, speech and occupational therapy for home bound clients as prescribed by their physician.

PCA Program:
The Personal Care Attendant Program is funded by medicaid and the State of Alaska general Funds to provide personal care attendant services to seniors with ongoing health concerns.  This program provides basic help with personal care such as bathing and dressing.

Project Choice:
This medicaid program provides comprehensive care coordination by a social worker or registered nurse.  The Choice Program is designed to give frail elders the choice to remain at home as an alternative to long term care.

Volunteer Visitor and Phone Friends:
These two programs connect volunteer community members with people who have become isolated due to illness or lack of mobility.  These volunteers develop a relationship with the client either by telephone or regular visits.

Diabetes Education:
South Peninsula Hospital is recognized by the American Diabetes Association for its Diabetes Self-Management Program. This program meets the national standards for excellence in diabetes education. South Peninsula Hospital offers quarterly classes on topics such as learning to live with diabetes, stress and coping, basics of eating and meal planning, physical activity and exercise, oral medications and insulin, long term complications, and behavior modification.
This educational program provides training, information, counseling and supplies for people living with diabetes.  The program focuses on becoming and remaining healthy.   A registered nurse and dietitian are available for home bound and outpatient clients.

Lifeline:
This emergency responder program provides peace of mind for frail home bound clients.   Lifeline is a telephone system that can be accessed by the push of a button in the event of an in-home emergency.
