South Peninsula Hospital's Home Health Services is a network of programs designed to meet the needs of elderly and disabled individuals choosing to remain at home in their community. We work closely with each client, their families, physicians, and other agencies to provide an individualized service plan for people of all ages. Home Health - We provide skilled nursing care, physical therapy, medical social work, certified nursing assistants and speech and occupational therapy for homebound patients as prescribed by their physician. CHOICE Waiver Program - The Choice Waiver program is designed for older Alaskans and individuals with disabilities. This program enables clients to remain in their home by providing comprehensive care coordination, chores, respite, environmental modifications, skilled nursing visits and specialized medical equipment to eligible recipients. Lifeline - With Lifeline, your loved ones can rest assured that you're safe, secure and happy in your home. Private Care Management - Specializing in Geriatric Care, we offer home care services on a long or short term basis to assist clients of all ages with personal care, housekeeping, shopping, errands, chores, nursing care, respite, companionship and personalized care management.
